CRM: Update Chart.js dependency to 4.5.0

This updates Chart.js to 4.5.0. There were no breaking changes, but some filenames changed and I cleaned up a bit of the code (e.g. moving JS content into the proper JS file).

I also adjusted the dash main chart header a bit to look better on mobile.
