Chapter 10 - Command Buffers

• Added chapter 10 (on Command Buffers) to the book
• Refactoring, fixes, etc per usual

Overall 3,974 additions and 3,199 deletions (a good chunk of that is project files being modified)

Major update

• Fixed chapter 4 not fitting in and not being as clearly written
• Fixed chapter 5 links (by removing and replaced with separate pages that sandwich the platform dependent code)
• Punctuation and link fixes
• Renamed VulkanExample to VulkanSwapchain

Overall, 665 additions and 506 deletions.
