Image not uploading when answering question

Describe the bug

When you try to upload any image when answering question, the image does not upload and rather pop with "File Cannot be empty" error

To Reproduce

Steps to reproduce the behavior:

1. Add a Question
2. Answer any question and try to upload image file from your computer
3. Click Add and you will get "File cannot be empty" error

Expected behavior

File should be uploaded when answering a question
